Our Coalition depends on the steady support of five organizations in Michigan all dedicated to making life better for our most challenging children and their families. Employees of these member organizations have access to the educational programs offered through our website.

Hospice of Michigan

The JoElyn Nyman Anchors Programs for Children is the pediatric arm of Hospice of Michigan.  Anchors Programs provide hospice care,  perinatal consultations, and a social work support program called the Compass Program.  Home hospice care is accessible by HOM’s nurses trained in pediatric care in most of the lower peninsula and the nursing staff partners with adult hospice teams in the more remote parts of the state to provide high quality care during prolonged illnesses and at the end-of-life.
Anchors Programs social worker Tara Derby is on our coalition’s board of directors.

C.S. Mott’s Children’s Hospital

The C.S. Mott Children's Hospital in Ann Arbor established the Pediatric Palliative Care Program in 2005.  This interprofessional team provides consultation from pregnancy through young adulthood, supporting goal-concordant care and complex symptom management in the inpatient and outpatient setting.  Mott is also home to the Partners for Children-Little Heroes League, a care coordination AND palliative care program supporting children with medical complexity.  This program was specifically designed to help keep children at home as much as possible by having 24/7 phone availability, regular home visits by members of the care team, and support while in the hospital. Mott' s Pediatric Care Program is led by Dr. Patricia Keefer.   Dr. Keefer is on our coalition's board of directors, as is retired nurse-educator Maureen Giacomazza who was a founder of the palliative care programs at the University of Michigan Hospitals.
